Please find attached the PISR forms
1 & 2 along with the NIRB screening table 1 (a signed copy will be faxed).
The word copy of the PISR form 2 that you had sent me contains a “draft” water
mark on the form; therefore I did not save it as PDF file. Let me know if
this will pose a problem.
As you had suggested, under the PISR
form 2 I referred to the relevant documents to answer the questions. Just to
ensure that you have all of the relevant documents, I am including the link to
Rescan’s FTP site where you can access all of the
documents.
To download the files, please follow the steps
below:
1. Open your web browser (Microsoft Internet Explorer or
Netscape Navigator) and direct it to http://www.rescan.com/QLclients.asp.
2. Select “Starfield Resources Inc.” from the list,
click "GO," and then choose the "KIA Applications" folder from the
list.
3. A dialog box will appear, prompting you to enter a
user name and password. Enter the user name “Starfield” and the password
“577-5”. Type both user name and password exactly as
shown.
4. You will see a list of subfolders. Select the
subfolder and you will see a list of files available for download from the
folder. Click the file name and, when prompted, save it to your hard disk
or network drive.
5. Close your browser and open Adobe Acrobat Reader (if
you do not have Acrobat Reader already installed on your computer, you can
download it for free from www.adobe.com/products/acrobat/readstep2.html).
Open the downloaded file in Adobe Acrobat Reader.

As promised, here are
the word versions of the NIRB Part 1 Application and NIRB Part 2 PSIR
Application.
As we spoke about on
the phone, feel free to reference another document in the PSIR Application if
the question/information has been already been provided. Just please make
sure that NIRB is in receipt of any documents that you
reference.
Also, it is not
necessary for you to complete the NIRB Part 1 Application in Inuktitut, your
project description translation is sufficient.
